Agrace Hospicecare

Agrace Hospicecare is a nonprofit, community-based health care agency founded in 1978 as HospiceCare Inc. It is Wisconsin's largest nonprofit hospice provider, serving 19 counties in southern Wisconsin including Dane, Rock, and Jefferson. The organization rebranded to Agrace in 2011 to distinguish its diverse service lines, which now include hospice, palliative care, adult day services, and a non-medical home care program called Age at Home. The Madison campus features the Don & Marilyn Anderson HospiceCare Center, a freestanding inpatient unit with 50 beds. Agrace is recognized for its specialized end-of-life care, offering unique facilities such as the Hospice Memory Care Suites—one of the few units in the country specifically designed for hospice patients with dementia. Their services are delivered by an interdisciplinary team of physicians, nurses, social workers, and spiritual counselors who focus on comfort, dignity, and quality of life. In addition to clinical care, Agrace operates several thrift stores whose proceeds support their charitable 'Care for All' program, ensuring that no one is turned away due to an inability to pay. The facility is nationally recognized for its quality, holding a 10/10 score on the CMS Hospice Care Index and accreditation from the Community Health Accreditation Partner (CHAP). It is also certified by the American Heart Association for its advanced cardiac care program in hospice and palliative settings. The Madison campus serves as a hub for community education on grief, death, and dying, providing extensive bereavement support to the public regardless of whether their loved one was an Agrace patient.
